1. When do physical forms need to be turned in and where?

Physical forms must be turned in prior to the first day of practice which is August 18. Turn the forms into the office or the coach.

Physicals must be dated after May 1, 2008 to be valid for the 08-09 school year. Please make sure that all required signatures are on the form.

1. What is the new concussion form that KSHSAA is requiring?

The new concussion waiver form needs to be signed before your student is allowed to participate in KSHSAA sponsored athletic events, as well as cheerleading. If the student needs a physical form to participate, then they also need to have the signed waiver. You can click the link below to print off the concussion form. Concussion Waiver
